New Patients

If your appointment is early in the morning please remember that the morning traffic around both offices is slow and additional driving time may be required. 

Parking

• Bella Vista Office: Free parking is available on the upper or lower podium levels.  If parking is not available on the upper podium, try the lower levels (accessed by the down ramp), you will need to take the stairs next to the up ramps to access our office, which is on the first floor of the Westpac bank building.

• St Leonards Office: Ample metered 1 to 2 hours of street parking is available around Pacific Highway, Berry Road or Park Road.

Please arrive 15 minutes before your appointment time to ensure that you have time if further paper work is required.

What to bring

• Your referral from your GP or healthcare professional
• Your medicare and private health insurance details
• Any Xrays you have had that are relevant
• Any other tests Hearing Tests, Balance Tests, Blood tests you may have had that are relevant
• The Norwest ENT Group New Patient and Health questionnaire, see below (if you have a printer, please print out these forms and fill them in prior to your arrival and bring them with you).
